Refer Patient screen: Referring Veterinarian Information section
Veterinarians can use the Refer Patient screen to refer patients to a specialty or emergency practice. The Referring Veterinarian Information section of the screen has information about the referring veterinarian and their hospital.
Note: Specialty and emergency practices can change the boxes that show in the Refer Patient screen. As a result, the sections can be different when you refer patients to different practices.
Note: Specialty and emergency practices can change which boxes are necessary for you to use. Each box that is necessary to use has an asterisk (*) adjacent to its name.

Note: If the necessary information is in your profile, rVetLink automatically enters the information in the boxes.

| Hospital Name | Use Hospital Name to enter the name of your hospital. |
| Veterinarian's Name | Use Veterinarian's Name to enter the name of the veterinarian who refers the patient. |
| Submitted By | If you refer the patient for a veterinarian, use Submitted By to enter your name. |
| Phone Number | Use Phone Number to enter the phone number of your hospital or the referring veterinarian. |
| Fax Number | Use Fax Number to enter the fax number of your hospital. |
| E-mail Address | Use E-mail Address to enter the email address of your hospital or the referring veterinarian. |
